LOCATION: Back To Life is located at 329 Bryant Street, Suite 2A, San Francisco, CA 94107.

We are in the south of Market district near the corner of Bryant and 2nd Streets. Back To Life is accessible by the #10 bus. The following lines come within 2 blocks of our office: N, T, 9X, 12, 45 and 7.

INSURANCE: Back To Life does not participate with insurance providers. Please check with your health insurance to see if they will reimburse you for your physical therapy visits with us.

APPOINTMENTS: Each appointment is 1 hour in length.

CANCELLATIONS: appointment cancellations require 48 hours (or 2 business days’ notice). Because a full hour is reserved for you, you will be charged the full rate for your appointment if you do not cancel your appointment in advance.
